Follow these four easy steps to participate in the Hearing Care Program.

1. Locate a Center near you. Confirm with the audiologist that he or she is a HearUSA Hearing Care Network provider and identify yourself as a member with HearUSA discounts.
   
   
2. Print and take the Discount Summary Form with you to your appointment. The network provider will complete the form and return it to HearUSA Hearing Care Network. (It is for reporting purposes only. It is not a claim for payment.)
   
   
3. Pay the network provider directly for services and products at the discounted charges.
